The service will support patients aged 18+ and over who live in South Tyneside and identify as trans or non-binary that require advice, guidance, or support. Patients can request a referral by contacting a member of the admin team on 0191 5190077.
The service shall:
· Provide one-to-one support, advice, guidance and information tailored to the needs of individual service users.
· Offer family support where appropriate and agreed with the service user.
· Undertake appropriate signposting and/or onward referral as necessary. This will include promotion and access to existing means of support and services for trans people.
· Provide age-appropriate peer support groups and access to social activities in order to enable service users to make connections, develop peer support networks and reduce their social isolation.
· Promote the health and wellbeing of service users including promotion of preventative interventions such as health screenings
· Deliver practical support and information, advice and guidance when required around gender identity and transitioning including signposting to GP for referral to Gender Identity Services,
· Provide interventions on coping strategies, self-care and promote the emotional wellbeing of service users.
· Identify needs which necessitate more specialist support.
· Build on community visibility and acceptance by working with other partners such as South Tyneside arts and cultural organisations, community events and campaigning for better understanding of trans people.
· Actively promote and publicise the service to people and partner agencies to raise awareness amongst service users and professionals.
· Work in partnership with GP’s and other primary care services.
