If you have a Power of Attorney document which names a family member or friend who will take over making decisions about your health if you no longer have capacity, please make this known to our reception team. They will ask to scan a copy of the document and who it names on to your medical record. This allows staff to speak to your nominated individuals should you lose capacity to make decisions for your health.
