South Tyneside Floating Support Service run by Mental Health Matters provides support for patients with mental health needs to gain and maintain their own homes.
Recovery Workers will help with skills such as:
* Staying safe at home
* Budgeting money and paying bills
* Applying for appropriate welfare benefits
* Developing life skills to live independently
* Maintaining physical and mental wellbeing
* Accessing the local community
* Shopping and cooking healthy meals
* Developing a peer support network
* Moving on to independent living
This service is for patients aged 18 and over who live in South Tyneside and are registered with a South Tyneside GP. They have no waiting list.
All referrals must be made by a Nurse, ANP, GP, social worker, CPN, drug and alcohol worker or other support worker. Please contact us to arrange an appointment if you would like us to arrange a referral.
