Information Sharing

Confidential personal data will be shared within the health care team for continuing care, unless you object. You may withdraw consent at any stage. When personal data is to be used for any other purpose, then explicit and fully formed consent will be obtained. Any information shared between the surgery and services such as the Primary Care Trust will be anonymised.

Confidentiality is owed equally to all patients regardless of their age, status or mental capacity.

Great North Care Record

South Tyneside and Sunderland NHS Foundation Trust has become the third trust to begin sharing patient information as part of the Great North Care Record programme.

Great North Care Record gives the staff caring for you a more complete picture of your medical history. It does this by making information held about you in different places such as your GP practice or local hospital available to the staff who are looking after you. Being able to access this information helps make your care safer.

The information available includes:

  • Patient details and demographics
  • Allergies and adverse reactions
  • Encounters
  • Problems and diagnoses
  • Procedures
  • Vital signs
  • Admissions and discharges
  • Laboratory results
  • Microbiology
  • Care plans and miscellaneous reports
